A statement, made under penalty of perjury, that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
Your full name, address, telephone number, and email address.
A statement that you consent to the jurisdiction of the federal district court for the judicial district in which your address is located (or, if you reside outside the United States, for any judicial district in which Epix IPTV may be found), and that you agree to accept service of process from the person who filed the original DMCA notice or their agent.
Your physical or electronic signature.
Once a valid counter-notification is received, we may restore the material in accordance with the procedures set forth in the DMCA.
4. Repeat Infringers
Epix IPTV reserves the right to terminate user accounts that are found to be repeat infringers or who repeatedly violate this policy or applicable copyright laws.
5. Misrepresentation
Please be aware that, under Section 512(f) of the DMCA, any person who knowingly and materially misrepresents that material or activity is infringing may be held liable for damages, including costs and attorney’s fees.
